What Is an Impacted Wisdom Tooth?

An impacted wisdom tooth is a third molar that fails to emerge fully into its correct position. Impaction is classified by how much of the tooth has erupted and the angle it takes.

  • Soft tissue impaction The crown has partially broken through the gum but the tooth cannot fully erupt, leaving a flap of gum tissue that traps bacteria and debris.
  • Partial bony impaction The tooth is partly submerged in the jawbone and partly through the gum. This is the most common type seen at our Istanbul clinic.
  • Full bony impaction The tooth remains entirely within the jawbone. Surgical access to the bone is required for removal.
  • Horizontal impaction The tooth lies on its side, pressing directly into the root of the adjacent molar. This position carries the highest risk of root resorption in the neighbouring tooth.

Why Wisdom Teeth Become Impacted

The human jaw has become smaller over thousands of years of evolution, yet the number of teeth remains unchanged. Wisdom teeth — the last to erupt, typically between the ages of 17 and 25 — frequently find insufficient space at the rear of the arch.

  • Jaw size and arch length Insufficient space at the back of the dental arch is the primary cause. This is hereditary in many cases.
  • Eruption angle A tooth that tilts mesially (toward the front) will press against the second molar rather than erupt vertically.
  • Thick overlying gum or bone Dense soft tissue or bone can physically obstruct upward movement even when space is available.
  • Early loss of adjacent teeth Paradoxically, losing a second molar can allow a wisdom tooth to drift into an unfavourable position.

Initial assessment
A digital panoramic X-ray is taken at the first visit. If the tooth is deeply positioned or close to the inferior alveolar nerve, a CBCT scan is recommended. The surgeon reviews the imaging with you and outlines the extraction plan.

Surgical preparation
The procedure is carried out under local anaesthesia as standard. The area is fully numbed before any instrument contacts the tissue. Most patients at our Istanbul clinic are surprised by how little they feel during the procedure itself.

Extraction
Depending on impaction type, the surgeon may need to reflect the gum tissue, section the tooth into smaller parts, or remove a small amount of the overlying bone. Horizontal and full bony impactions typically require bone sectioning.

Recovery

Recovery: What to Expect After Wisdom Tooth Surgery

Recovery from impacted wisdom tooth extraction in Istanbul typically progresses through predictable stages. Individual experience varies depending on impaction type, the number of teeth removed, and patient-specific factors.

Days 1–2
Swelling and mild bruising around the jaw are normal and typically peak at 48–72 hours. Biting on gauze immediately after the procedure helps initial clot formation. Keep the head elevated when resting.
Days 3–5
Swelling begins to reduce. Soft foods — soups, yoghurt, mashed foods — are recommended during this period. Avoid rinsing forcefully, drinking through straws, or smoking, as these can dislodge the blood clot and increase the risk of dry socket.
Week 2
Most patients are comfortable returning to normal eating and activity. Sutures are removed at the follow-up appointment if non-resorbable sutures were placed.
Weeks 3–4
Soft tissue heals fully. Bony healing of the socket continues over several months but is not noticeable in daily life.

There is no upper age limit, but the procedure is generally less complex in younger patients (late teens to early twenties) because the roots are not yet fully formed and the surrounding bone is less dense. In older adults, the roots are typically longer and more curved, and the surrounding bone is denser, which can make surgical extraction more involved. This does not prevent extraction at any age — it simply informs the surgical planning and recovery expectations.
